Referral Workflow
VTCPAP receives a therapy referral request from a PCP
A Liaison Coordinator (LC), who is also a Licensed Clinical Social Worker, will search through available therapists to match the family with a provider who has training in an evidence-based modality to address the identified need.
The LC team will often reach out to 15-20 providers to try to find an appropriate “match” who has current or anticipated openings, utilizes an evidence-based modality, serves the targeted population, and accepts the family’s insurance.
The LC team will provide potential therapists with a brief summary of the presenting clinical and demographic information to work to find a good match between therapist and client.
The LC will try to respond to the PCP within 24-48 hours of the call to give them a few options for the family to reach out to.
